Why this matters in South Africa

Tuberculosis is the leading cause of death among people living with HIV, and the two infections feed each other. HIV weakens the immune system enough for latent TB to become active, while TB speeds up HIV disease. In people with both, symptoms are harder to read, and TB is more likely to be missed on standard sputum tests.

Care has often been split: a TB clinic in one room, an HIV clinic in another, sometimes on different days with different records. Every extra visit is a chance for a patient to drop out, especially for people who work long hours, live far from a facility or face stigma. Those most affected include mineworkers, residents of informal settlements, prisoners and people who use drugs. Treating both infections in one place, with one care team and adherence support between visits, keeps patients on both courses of treatment and prevents deaths that occur when one condition goes untreated.

What your gift pays for

  • Combined TB and HIV testing at a single visit, including rapid molecular tests for TB
  • Adherence counselors who follow up patients managing two long treatment courses at once
  • Home visits and tracing for patients who miss appointments or stop collecting medication
  • TB preventive therapy for people living with HIV who do not yet have active disease

Organization

TB HIV Care · South Africa

Founded 1929 · Headquarters: Cape Town

South African NPO providing integrated TB and HIV prevention, testing and treatment services.

How they work: TB HIV Care began as a TB-focused organization in Cape Town and now works in several provinces. Alongside clinic-based support, it runs community programs that bring testing and treatment to groups often missed by mainstream services, including people who use drugs, sex workers and people in correctional facilities, using outreach teams and peer educators. Its staff work with Department of Health facilities on case finding, starting treatment and keeping patients in care.

Track record: With CDC/PEPFAR support since 2007, TB HIV Care has delivered TB and HIV diagnosis, treatment and adherence support in high-burden districts of three South African provinces.

Giving in South Africa

Inequality between townships and the rest of South Africa is the thread through its four causes. Formal unemployment stands at 33.6 percent, the highest of any G20 economy, and the funds respond with surplus food delivered to community kitchens serving informal settlements and daily meals for township children. On the health side, they support integrated treatment for people co-infected with TB and HIV and staffing for overstretched public clinics. Partners are South African organizations such as FoodForward SA and TB HIV Care.

4 causes are listed in South Africa, each with its own verified organization. $394,893 has been distributed to causes in South Africa since 2021.
