Why this matters in the United States

Illicitly manufactured fentanyl now drives most overdose deaths, often pressed into counterfeit pills or mixed into other drugs without the buyer knowing, and increasingly combined with the veterinary sedative xylazine. Those dying are not a single group: they include young adults who took one pill, long-term users, and people recently released from prison or detox whose tolerance has dropped. Rural counties and Black and American Indian communities have seen some of the highest death rates in recent years.

Naloxone reverses an opioid overdose, and the FDA approved a naloxone nasal spray for over-the-counter sale in 2023. A pharmacy shelf is not the same as the right pocket at the right moment, though. Many people at highest risk do not visit pharmacies, cannot afford a kit, or fear contact with police. Community distribution by outreach workers, syringe services and peers puts naloxone in the hands of the people most likely to witness an overdose, along with the training and fentanyl test strips that go with it.

What your gift pays for

  • Naloxone kits handed directly to people who use drugs, their families and outreach teams
  • Overdose response trainings for community groups, shelters and frontline workers
  • Fentanyl test strips and other safer-use supplies distributed alongside naloxone at outreach sites
  • Peer outreach staff who connect people to medication treatment such as buprenorphine or methadone

Organization

National Harm Reduction Coalition · United States

US IRS EIN · No. 94-3204958

Founded 1994 · Headquarters: New York City

US nonprofit advancing overdose prevention, including naloxone distribution training and harm reduction programs.

How they work: National Harm Reduction Coalition works largely by building other organizations' capacity. It trains health departments, community groups and frontline staff in overdose response and harm-reduction practice, produces training materials, and convenes the national harm reduction conference that brings practitioners and people who use drugs together. Its San Francisco program shows the direct-service side, with trained staff and peers distributing naloxone through syringe programs, shelters and other community sites. The coalition also pushes for policies that widen access to naloxone and treatment.

Track record: Runs the DOPE Project in San Francisco, described as the largest single-city naloxone distribution programme in the US, and delivers hundreds of overdose prevention trainings each year.

Giving in the United States

Where public systems leave gaps, the American causes on this site step in: rural towns that have lost their only hospital or cannot afford to fix aging water lines, cities where rents have outrun shelter capacity, and counties hit hardest by opioid overdoses. Other funds back veterans, survivors of domestic violence, foster youth aging out of care and Hispanic college students, many of them first-generation. Partners range from national nonprofits such as Meals on Wheels America and the American Red Cross to a federal rural health office and the Department of Veterans Affairs.

24 causes are listed in the United States, each with its own verified organization. $2,628,640 has been distributed to causes in the United States since 2021.
