Why this matters in Japan

Close to three in ten residents are now aged 65 or older, the highest share of any major economy. A study group commissioned by the health ministry estimated that 4.43 million older people were living with dementia in 2022, about 12.3% of that age group, and projected roughly 5.87 million by 2050. Many more have mild cognitive impairment, the stage at which treatment and risk reduction may help most.

Families still provide much of the daily care, and the carers are aging too: spouses in their eighties looking after each other, or adult children leaving work to care for a parent. Long-term care insurance pays for services, but care facilities and home-care agencies struggle to hire staff. Antibody drugs approved in recent years work only in early Alzheimer's and require confirmation of amyloid in the brain, which makes early detection and trial-ready research cohorts far more valuable. Research on prevention, diagnosis and care models feeds directly into national dementia policy.

What your gift pays for

  • Follow-up assessments for registry participants who may join future prevention trials
  • Brain imaging and blood biomarker analysis for early detection research
  • Studies of caregiver support and community care models for people living at home
  • Data management that lets research sites share standardized clinical records

Organization

National Center for Geriatrics and Gerontology (NCGG) · Japan

Government body

Founded 2004 · Headquarters: Obu

Japan's national research centre for aging, a leading public institute for dementia research.

How they work: NCGG, in Obu, Aichi Prefecture, combines a research institute with a hospital that runs a specialist memory clinic, so studies can draw on patients seen in routine care. Beyond the Orange Registry, it has led multidomain prevention research such as the J-MINT trial, which tests combined exercise, cognitive training and nutrition programs in older adults with mild cognitive impairment. Its researchers also work on biomarker-based diagnosis and on support programs for families and community-based care.

Track record: NCGG runs the Orange Registry, a longitudinal cohort registering approximately 2,000 healthy and preclinical Alzheimer's individuals a year for future trials (AMED Symposium 2017).

Giving in Japan

Demographic change lies behind every Japanese cause on the list. Rural seniors living alone on fixed pensions receive subsidized meal delivery and regular home visits, in areas with the country's highest rates of unattended deaths among older people. The National Center for Geriatrics and Gerontology conducts dementia research as case numbers rise with an aging population. Florence, a Japanese nonprofit, expands subsidized childcare to ease a shortage of caregiver capacity that leaves some mothers isolated.

4 causes are listed in Japan, each with its own verified organization. $364,226 has been distributed to causes in Japan since 2021.
